Job Description:
Provide emotional and practical support to clients facing social, personal, or economic challenges
Assess client needs and develop individualized support plans
Assist clients in accessing community resources such as housing, financial aid, healthcare, and counseling services
Support clients in developing life skills (budgeting, communication, daily living skills, etc.)
Maintain accurate case notes, progress reports, and documentation
Advocate on behalf of clients to ensure their needs are met
Collaborate with social workers, healthcare providers, and community agencies
Monitor client progress and adjust support plans as needed
Ensure all services are delivered respectfully and in accordance with organizational policies and confidentiality requirements
Job Requirements:
Diploma or degree in Social Work, Community Support, Psychology, or a related field
minimum 1 year working experience in community support, social services, or client-focused environments
Strong communication, problem-solving, and interpersonal skills
Ability to work independently and as part of a team
Knowledge of local community resources and support programs
Valid driver’s license
Current CPR/First Aid certification (an asset)
